I am aware that REN XUE was designed to be a safe, effective and efficient alternative modality for health and wellbeing, but it also has its limitations.

To my best knowledge, I have no medical or psychological condition that would prevent me from taking part in this REN XUE activity, and I assume all responsibility for my participation. I understand that REN XUE and Yuan Qigong are not a substitute for medical attention, examination, diagnosis, or treatment.

I have read, fully understand, and agree to the terms of this Informed Consent Agreement and Supporting Information. I indemnify and release Ren Xue of the Americas, Yuan Tze, and all people operating on their behalf, from all liability for personal damage or injury to myself. I am signing this agreement voluntarily and recognize that acceptance of these terms serves as complete and unconditional release of all liability to the greatest extent allowed by law.

I hereby give my consent to REN XUE of the Americas, its affiliates and agents, to use my image and likeness and/or any interview statements from me in its publications, advertising or other media activities (including the Internet) without expectation of compensation or other remuneration, now or in the future. This consent includes, but is not limited to:(a) Permission to interview, film, photograph, tape, or otherwise make a video reproduction of me and/or record my voice; (b) Permission to use my name; and c) Permission to use quotes from the interview(s) (or excerpts of such quotes), the film, photograph(s), tape(s) or reproduction(s) of me, and/or recording of my voice, in part or in whole, in its publications, in newspapers, magazines and other print media, on television, radio and electronic media (including the Internet), in theatrical media and/or in mailings for educational and awareness.
