I have read the information supplied about Looking After Your Mental Health, and to the best of my knowledge, I have no medical or psychological condition that makes taking part in the RXA Qi Hive not suitable for me at this time – and I assume all responsibility for my participation. I understand that REN XUE and Yuan Gong are not a substitute for medical attention, examination, diagnosis, or treatment. I agree that all personal information of any participation shared within the event is strictly and completely confidential. I shall not disclose any of this confidential information to any other person or entity under any circumstances. I agree that I shall not make any audio, video, audiovisual, photos, or other recordings of any portion of the workshop nor shall I duplicate, reproduce, distribute, publish or discuss any of the confidential information contained in or related to the Qi Hive Sessions. I have read, fully understand, and agree to the terms of this Informed Consent Agreement. I indemnify and release REN XUE of the Americas, REN XUE International, all people and organizations operating on their behalf, from all liability for personal damage or injury to myself.
